Please complete the following forms to register your business in our supplier database. Our Supplier Outreach Team uses this database and the profiles in it to identify suppliers for business opportunities.

• Supplier Code of Conduct
• Vendor Information Form
• W-9 Form
• Insurance Requirements
• Electrical Contractor Project Reference Form (electrical contractors only)

Please email the completed forms to the requesting Cal Water manager or to [email protected]. Alternatively, you can fax them to (408) 392-9797, or mail them to California Water Service, Attn: Procurement Department, 1720 North First Street, San Jose, CA 95112-4508.

By submitting the Vendor Information and W-9 forms, you hereby accept Cal Water’s Terms & Conditions for doing business with us.
